Online Athletic Registration:

Use the Link below or go to Tacoma Public Schools Mainpage and scroll down until you see "Athletics Registration"

-Click on the large 'Register Online" button. You can enter your HAC credentials and the form will fill in your address and any known information about you or you can proceed to a blank form and fill this information out manually. (You must know your students school ID number to fill out form) Both of these methods will work just fine.

-The online registration will walk you through the entire on-line registration process, replacing the need to collect paper copies of the following:

-District Athletic Contract. Both parent and student agree to the rules and regulations of the Tacoma Public Schools

-Athletic Registration Form. Parent gives permisssion for student to play athletics.

-Concussion and Sudden Cardiac arrest awareness forms. The explanation of coach, officials, and school AD actions. the bottom line is that if we suspect the child has a head injury, the athlete will not participate until he/she has been seen by a doctor and given release papers. This is done for long term safety of all athletes.

-Informed consent form.

-Emergency card information (coaches carry this so parents can be informed if a child was hurt).

Electronic acknowledgement of the above forms will replace a formal signature that is required on the paper form. All forms must be filled out entirely.

A copy of your child's physical is still required and lasts 13 months. Requirements include a doctors stamp , signature, date, and the words "cleared without restriction". Doctors forms can be downloaded using the link below.

Athletic eligibility

To be eligible to participate, students will need the following items:

-Valid Sports physical (Tacoma School District honors physicals for 13 months) The focus of my attention when accepting a physical is the date and the words "Cleared without Restriction".

-Middle School Activities Contract and Eligibility Form. This includes an insurance form that the parent must fill out stating the athlete has insurance (Athletes must have health insurance). This is filled out during on-line registration.

-Emergency Medical Card. This is filled out during on-line registration.

-Concussion information form. This is filled out during on-line registration.

-ASB Card purchased. Cost is $15 and is purchased in main office.

-Students must maintain a 2.0 GPA with no failing grades. Grades are checked at least every other week. Students can bring grades up to be eligible.

-Honor Level 1 or 2 to participate. Honor Level 3 means an athlete can practice but not play in a game. Honor Level 4 means athlete cannot practice or play in games until Honor Level improves (Honor Level is affected for 2 weeks from infraction).
